Can the same menu be in the pause menu and Quick Actions?

Yes. pause-menu and quick-actions are separate boolean settings and can both be enabled.

Why does Quick Actions not open?

The vanilla Quick Actions key bind has no default key. Players must bind it in Options → Controls → Key Binds → Miscellaneous → Quick Actions.

What can /custommenu reload change?

Runtime actions/messages/menu targets and inline menu layout reload. Pause/quick-action registry layout and the open-command name need a restart.

Do I need ItemsAdder or Nexo?

No. Both are optional and only needed if your menu text uses their custom glyphs.
